Welcome new patients and returning patients. We have provided you with this information to make your registration time more efficient and to prepare us to better serve you. Please print, fill out and bring the forms listed below with you for your appointment. You may be asked to arrive earlier than your scheduled appointment time to allow for entry of your information into our computerized system.
We realize that every patient is unique and has specific needs; therefore, we are committed to giving you the personalized attention and care that you deserve. We strive to see our patients in a timely fashion, but an occasional circumstance may arise that could cause a delay in your appointment. We do recognize and value the importance of your time. Please allow us to apologize in advance for any appointment delays.
What to bring on your first visit
You can request refills via email or phone call. We have a nurse that returns emails and calls daily for prescription refills and to answer established patient questions. If you have been seen by one of our physicians within the last year, refills can be called in. If you are sick, you will need to make an appointment. Please email the nurse via your patient portal or call our office during regular business hours, 662-844-6513 ext. 2229 for the nurse.
Information needed for Refills
Please check with your pharmacy the following day. The nurse will only call you back if there is a problem with your refill.
Thank you for choosing ENT Physicians of North Mississippi to serve your healthcare needs. We value the importance of your time. As a courtesy to you and our other patients, we ask that you arrive on time for your appointment.
We realize that occasional extenuating circumstances may arise that could delay your arrival time; therefore, we do make every attempt to work with you. When possible we make an effort to work in our patients when they arrive up to 20 minutes late for their appointment. If arrival is past this time, the receptionist will reschedule your appointment.
As a courtesy to our patients, we call daily to confirm appointments. If you know that you will be unable to keep your appointment, you can cancel and reschedule at that time. If the need to cancel your appointment arises before or after the confirmation call, please call our office in advance so that we may accommodate the acute needs of patients calling in for appointments.
If you have seen one of our physicians and have discussed the need to schedule surgery, we realize that you may have additional questions; therefore, we have provided this information to assist you.
Our physicians and surgery nurse will discuss the details of your procedure at your pre-operative appointment, including: scheduling a date for the surgery, an estimated surgery time, the location, your follow up appointment, as well as your post operative instructions.
We will contact your insurance company. It is always a good idea for you to call your insurance company as well.
